An inquest into Ruth Perry's death found there was a 'link' to the inspectionThe ex-head of Ofsted has defended her decisions during the Ruth Perry tragedy - saying the watchdog has to give out 'tough messages' even if it upsets people.
Ofsted faced scrutiny following the tragic death of 53-year-old Mrs Perry , who was left 'completely devestated' after inspectors downgraded her primary school to 'inadequate' over safeguarding concerns And she added that too often the focus is about 'being nice to the adults' working in public services 'at the expense of the service users'.

'The thing I've found people are incredibly unwilling to acknowledge and discuss is that there is no possible way that you can ever make everyone perfectly happy and totally protect the interests of children, and make sure that you never have to say anything to an adult that could disappoint or upset them.
Ms Spielman left Ofsted in December 2023. She had previously apologised to Mrs Perry's family for the distress she suffered
